About the event
Bipolar disorder is not another name for “mood swings.”
Clinicians rarely miss the depression. Mania and hypomania are where diagnostic reasoning more often begins to wobble. Irritability, reduced sleep, impulsive behavior, and intense emotional reactions may appear in bipolar disorder—but none is specific enough to establish it. A person who cannot sleep is not necessarily experiencing a decreased need for sleep. Rapid, “on and off like a light switch” reactivity does not have the temporal quality of a sustained manic episode. And not every impulsive purchase represents the kind of consequential risk-taking associated with mania.
Trauma can complicate the picture even further. Hyperarousal and trigger-linked reactions may be severe, dangerous, and genuinely life-altering while still being fundamentally different from mania. Screening for PTSD is not an optional side trip in the differential diagnosis; without it, clinicians may recognize the intensity of the presentation while misunderstanding what is driving it.
